This year, parents/guardians who wish to exempt their child(ren) from school-required immunizations for personal belief reasons must take the ADHS online Immunization Education Course. Completing this course will generate a personal beliefs exemption form which indicates that they have viewed the required education, which the parent then signs and provides to the school. This must be completed prior to the first day of school, July 20, 2020 or your students cannot be enrolled until it is submitted.
If you student only needs their immunizations updated, you do not need to complete this course, however, their immunizations must be completed prior to the first day of school, July 20, 2020 or your student cannot be enrolled until this is completed. If you do not have insurance or the expense is a concern, you can call the Yavapai County Health Department at 928-771-3122 and they will provide them at no cost. Please send a copy of the shot record to the school when completed.
